Glucose & insulin
HOMA-IR
Calculated index of insulin resistance from fasting glucose × fasting insulin.

Calculated
Biology
What it measures
HOMA-IR = (fasting insulin µIU/mL × fasting glucose mg/dL) / 405 (or × glucose mmol/L / 22.5).
Clinical use
Why we order it
Quantify insulin resistance in non-diabetic adults; track response to lifestyle/pharmacologic intervention.
Lens · Clinical
Interpretation by lens
Clinical interpretation
Non-diabetic adults typically <2.0; >2.5 suggests insulin resistance; >5 is severe.
Functional interpretation
Functional target <1.0 in healthy lean adults; combined with fasting triglycerides/HDL ratio adds power.
Differential
Causes of abnormal values
Causes of HIGH
- ↑Insulin resistance
- ↑Metabolic syndrome
- ↑PCOS
- ↑NAFLD
Causes of LOW
- ↓Insulin-deficient diabetes
- ↓Fasting/cachexia
